Health Home services for children

OLV Human Services care management services follow a Home Health model to coordinate all services for children enrolled in our program, including behavioral health, medical and social services, and other care.  Our professional staff works to see that all of a child’s caregivers communicate with one another so that the patient’s needs are addressed in a comprehensive manner.

Home Health services are provided through a network of organizations, health plans and community-based providers. Our experienced care managers oversee and provide access to all of the services a child needs to remain healthy and stay out of the emergency room or hospital. Our coordination makes it easier for people with chronic conditions to receive the right combination of preventive services to improve overall health and reduce healthcare costs.

Program Highlights

  • Children from birth to age 21 with active Medicaid and two or more chronic health conditions are eligible to enroll
  • Children with single qualifying conditions may also be eligible including severe emotional disturbance, complex trauma and HIV/AIDS.
  • Care managers work with family and child to provide services such as

 

-Comprehensive Care Management

-Comprehensive Transitional Care

-Community and social support

 

-Care Coordination and Health Promotion

-Member and family support

 

  • Program works with Children’s Health Home of Upstate New York (CHHUNY) and Encompass to provide care management services
  • Care manager is key point of contact in managing services for the child
  • Enables constant communication and eliminates redundancy of services
  • Expansive network gives families access to long list of providers and care managers
