Opening roads to recovery from mental illness and substance use disorders

 

OLV Human Services is an Adult Behavioral Health Home and Community Based Service provider in Erie and Niagara counties. Our Adult HCBS provide opportunities for adult Medicaid beneficiaries with mental illness and/or substance use disorders to receive services in their own home or community.

Our services help to create an environment where managed care plans, service providers, plan members, families, and government partners can assist members in preventing and managing chronic health conditions and recover from serious mental illness and substance use disorders. Dedicated professionals and staff members include trained vocational specialists with experience working with individuals with barriers specific to mental health.

Service Highlights

  • Psycho-Social Rehabilitation – we help the individual develop skills necessary for compensating or eliminating functional deficits and interpersonal and or environmental barriers.
  • Habilitation Services – we assist individuals in acquiring, retaining, and improving skills necessary to reside successfully in their home and community
  • Pre-vocational – we provide person-centered services to help acquire the skills needed to obtain and retain competitive employment in the community.
  • Intensive Supported Employment – we help with employment plans, goal-oriented job development, support in the job acquisition process, as well as supports in the initial stages of employment
  • Education support services – we help individuals who want to start or return to school or formal training with a goal of achieving skills necessary to obtain employment.
  • Ongoing Supported Employment – we help those who have obtained and maintained community employment and need continued, but less intensive supports.

 

Program Highlights

  • Services may be provided to individuals who are eligible for enrollment into a Health and Recovery Plan (HARP) plan.
  • HARP enrollment is open to Medicaid beneficiaries age 21 and older with serious mental illness and/or substance use disorders.
  • HARP members will be required to be assessed for BH HCBS eligibility using the New York State (NYS) Community Mental Health Eligibility Assessment.
  • The NYS Eligibility Assessment will determine if an individual is eligible for BH HCBS, and if eligible, eligibility for Tier 1 or Tier 2 BH HCBS services.
  • Tier I services include employment, education, and peer supports. Tier 2 includes the full array of services.
